Hospital Transitional Support Services offers short term transitional services and supports to children and youth (up to age 18) who are inpatients on the pediatric mental health units at North York General Hospital and Humber River Regional Hospital or are involved in the day programs at either of these hospitals.
The hospital transitional support worker is located at the hospital and receives referrals from hospital personnel to provide supports to children and youth as they transition out of the hospital back to their home and communities. Hospital transition workers will assist children, youth and families in facilitating referrals to mental health services in their communities.
The hospital transitional support worker works as part of the hospital’s multidisciplinary team to develop a transitional service plan that best suits the needs of the child/youth and family.
